Provide compassionate, culturally sensitive diabetes education to
patients and families, translating complex care into doable steps
Utilize the nursing process to assess, plan, implement and evaluate
individualized patient care plans Administer prescribed medications
and treatments safely and in line with state law and facility
policies Document care accurately and according to standards to
ensure continuity and quality of care Collaborate with an
interdisciplinary diabetes management team to support longitudinal
patient outcomes Act as a resource for staff on patient education
materials and program development Participate in unit performance
improvement and shared governance activities to drive better care
Advocate for patients addressing clinical, psychosocial and
care-coordination needs Qualifications Licensed Registered Nurse
(RN) in Illinois (IL) and Wisconsin (WI) — both required CDCES
(Certified Diabetes Care and Education Specialist) — required
Approved CPR certification (obtainable within 90 days of hire)
Graduate of an accredited nursing program (BSN preferred)
Comfortable administering medications, developing care plans, and
documenting per standards Willingness to float to other units and
collaborate within multidisciplinary teams Strong critical thinking
and patient advocacy skills Education Graduate of an accredited
nursing program (BSN preferred) Knowledge & Skills Diabetes
education techniques and patient/family teaching strategies
Clinical assessment and the nursing process for individualized care
planning Medication administration and safe practice standards
Documentation best practices and clinical record-keeping
Interdisciplinary collaboration and communication Performance
improvement and shared governance principles Compassionate,
